Below are all the forms that you need to print, fill out and bring with you to your first appointment. If you cannot do this, please arrive 15-20 minutes early to fill them out. If you are interested in filling all the forms out directly online, please call us at 972.347.1320 and request a link to be sent to you.
As part of your new patient paperwork, we need you to send us a copy of your Driver’s License and the front and back of your Insurance Card. Please make sure they are legible so that we can make sure your information can be entered into our system correctly. The form to upload these files is below.
